THIS TERMS OF SERVICE AGREEMENT IS EFFECTIVE AS OF 06/15/2026.

ACCEPTANCE OF TERMS

The following Terms of Service Agreement (the “TOS”) is a legally binding agreement that shall govern the relationship with our users and others which may interact or interface with Life Wellness Lab, LLC, located at [BUSINESS MAILING ADDRESS] and our subsidiaries and affiliates, in association with the use of the Life Wellness Lab website, which includes https://lifewellnesslab.com, (the “Site”) and its Services, which shall be defined below.

DESCRIPTION OF WEBSITE SERVICES OFFERED

The Site is a health and wellness services website which has the following description:

Provide information to consumers about the health, wellness, nutrition, and related coaching and consulting services offered by the company and ways in which consumers can contact the company to inquire about or request services.

Any and all visitors to our site shall be deemed as “users” of the herein contained Services provided for the purpose of this TOS.

The user acknowledges and agrees that the Services provided and made available through our website and applications, which may include some mobile applications and that those applications may be made available on various social media networking sites and numerous other platforms and downloadable programs, are the sole property of Life Wellness Lab, LLC.

At its discretion, Life Wellness Lab, LLC may offer additional website Services and/or products, or update, modify or revise any current content and Services, and this Agreement shall apply to any and all additional Services and/or products and any and all updated, modified or revised Services unless otherwise stipulated. Life Wellness Lab, LLC does hereby reserve the right to cancel and cease offering any of the aforementioned Services and/or products. You, as the end user acknowledge, accept and agree that Life Wellness Lab, LLC shall not be held liable for any such updates, modifications, revisions, suspensions or discontinuance of any of our Services and/or products. Your continued use of the Services provided, after such posting of any updates, changes, and/or modifications shall constitute your acceptance of such updates, changes and/or modifications, and as such, frequent review of this Agreement and any and all applicable terms and policies should be made by you to ensure you are aware of all terms and policies currently in effect. Should you not agree to the updated, revised or modified terms, you must stop using the provided Services forthwith.

Furthermore, the user understands, acknowledges and agrees that the Services offered shall be provided “AS IS” and as such Life Wellness Lab, LLC shall not assume any responsibility or obligation for the timeliness, missed delivery, deletion and/or any failure to store user content, communication or personalization settings.

PRIVACY POLICY

Every user’s registration data and various other personal information are strictly protected by the Life Wellness Lab Online Privacy Policy (see the full Privacy Policy at https://lifewellnesslab.com/privacy-policy). As a user, you herein consent to the collection and use of the information provided, including the transfer of information within the United States and/or other countries for storage, processing or use by Life Wellness Lab, LLC and/or our subsidiaries and affiliates.

SMS Messaging & Mobile Terms of Service

Sender Identity and Messages You Can Expect

Life Wellness Lab, LLC sends SMS and MMS text messages to individuals who have opted in to our messaging program. These messages may include appointment reminders, scheduling confirmations and updates, intake and onboarding follow-ups, and other service-related notifications, and — for those who separately opt in — promotional messages about special offers, discounts, and service updates. By providing your mobile phone number and checking the applicable consent box, you consent to receive these messages. Consent to receive SMS messages is not a condition of purchase or of receiving services.

Opt-Out and Support

You can cancel the SMS service at any time. Just text “STOP” to [BUSINESS PHONE]. After you send the SMS message “STOP” to us, we will send you an SMS message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from us. If you want to join again, just sign up as you did the first time and we will start sending SMS messages to you again. If you are experiencing issues with the messaging program you can reply with the keyword HELP for more assistance, or you can get help directly at [email protected].

Carrier Liability

Carriers are not liable for delayed or undelivered messages.

Message Frequency and Rates

As always, message and data rates may apply for any messages sent to you from us and to us from you. You will receive recurring messages, with frequency varying based on your interactions with us (for example, appointment reminders and scheduling updates as needed, and — if you have opted in to marketing — promotional messages no more than a few times per month). If you have any questions about your text plan or data plan, it is best to contact your wireless provider.

Age Restriction

You must be 18 years of age or older to participate in SMS communications.

CAUTIONS FOR GLOBAL USE AND EXPORT AND IMPORT COMPLIANCE

Due to the global nature of the internet, through the use of our network you hereby agree to comply with all local rules relating to online conduct and that which is considered acceptable Content. Uploading, posting and/or transferring of software, technology and other technical data may be subject to the export and import laws of the United States and possibly other countries.
